How to digitize med spa consent forms
Most clinics do not have a consent problem, they have a retrieval problem. The form was signed; nobody can find it two years later. Digitizing fixes retrieval first and speed second.

Digitize med spa consent by listing every form you currently use, rebuilding each as a template in your clinic software, mapping which service triggers which form, sending intake to clients before the appointment, capturing an electronic signature at or before check-in, and storing the signed document on the client record against the visit it covers.
1. Inventory what you actually use
Collect every form in circulation: general intake, medical history, treatment consent per service, photography consent, aftercare acknowledgements, and any clinic-specific paperwork.
You will usually find duplicates and versions that differ between providers. Decide which version is current before you rebuild anything.
2. Rebuild forms as templates, not PDFs
A scanned PDF is still paper. Rebuild each form as a structured template with real fields so answers land on the record as data you can read, filter and report on.
Keep photography consent as its own template rather than a checkbox inside a treatment form — it governs a different use and often a different duration.
3. Map forms to services
Attach each template to the services that require it, so booking a service automatically requests the right paperwork instead of relying on the front desk to remember.
New services added later inherit the mapping you define, which is what keeps the system accurate a year in.
4. Send before the visit and sign electronically
Intake completed at home is more accurate than intake completed in a waiting room, and it gives the provider time to read medical history before the appointment starts.
Capture the electronic signature at completion or at check-in, and store the signed document against the client and the specific visit.
5. Decide what happens to the paper archive
Scan and attach historical consent to the relevant client files where it matters, and agree a retention policy for the physical originals with your own advisers.

Common questions
- Are electronic signatures acceptable for med spa consent?
- Electronic signatures are widely used in clinical settings, but acceptability depends on your jurisdiction and your own professional and insurance requirements. Confirm with your advisers; software cannot make that determination for you.
- Should photo consent be a separate form?
- Usually yes. Photography is a distinct use of a client's likeness, often with a different scope and duration than the consent to treat, so it is cleaner to capture and store it separately.
- How long does digitizing consent take?
- For most clinics the work is a day or two of template building, dominated by deciding which version of each form is current rather than by the build itself.
