Why Consent Without Version History Is Not Evidence

Why Consent Without Version History Is Not Evidence

A signed consent form proves very little if you cannot show which version was signed, when, and that it covered the treatment actually delivered.

Start by mapping consent to treatment type with a validity period — per visit, per course, or annually for repeat patients. Outstanding forms go out with the appointment confirmation so the patient reads them on their own device rather than in a waiting-room rush.

Then make it a gate. If consent is missing or expired the chart cannot be completed, which surfaces the gap at check-in instead of during an audit months later.

Revisions create new versions; they never rewrite past signatures. And photography permission stays separate from treatment consent, so a patient can agree to clinical images while refusing marketing use — and revoke that use later without touching the clinical record.
