The Anatomy of a Defensible Treatment Record

Most clinic software records that a treatment happened. That is a receipt, not a record. A defensible chart states what was used, where, how much, from which lot, by whom, and under whose supervision.

Structure matters per modality. Neurotoxins chart per area in units with the dilution used. Fillers chart per syringe with product, plane and volume. Lasers chart passes, fluence and spot size against Fitzpatrick type. Facials and peels chart the protocol steps actually completed.

Inventory should move as the chart is written. Each product line draws from an open vial or syringe carrying its lot number and expiry, which turns a recall notice into a single query rather than an afternoon with paper files.

Finally, the record locks on signature. Corrections become amendments with author, timestamp and prior value retained. Aftercare goes out immediately and the follow-up interval is scheduled from the chart itself.
